Senior Staff Nurse

Job Responsibilities

Key Responsibilities
Clinical Assessment & Care Planning
• Conduct comprehensive admission and periodic assessments using standardized tools (e.g., Modified FRAT, FAST).
• Develop, implement, and update individualized care plans via the Elder & Community Caresuite.
• Perform critical reassessments following hospitalizations, falls, or significant absences.

Medication & Clinical Management
• Oversee medication management, including preparation, packing, and administration.
• Ensure strict adherence to the IDDSI framework for residents with swallowing difficulties (dysphagia).
• Respond promptly to medical emergencies and maintain rigorous infection control standards.

Leadership & Supervision
• Supervise and train care staff in delegated nursing procedures, enteral feeding, and medication support.
• Liaise with the Centre Lead regarding nursing equipment and day-care operations.
• Lead quality improvement initiatives to enhance resident safety and well-being.

Coordination & Communication
• Collaborate with a multidisciplinary team to ensure seamless care coordination.
• Maintain effective clinical communication with families regarding client conditions and care goals.
• Facilitate internal and external referrals to support holistic resident needs. 

Job Requirements

Academic & Professional Qualifications

• Diploma in Nursing or Bachelor Degree in Nursing

• Licensure: Valid registration with Singapore Nursing Board (SNB)
