Job Responsibilities

 OBJECTIVE(S)

  • To support the work of the Active Ageing Centre (AAC) and assist in fulfilling the objectives of the AAC and its intended purposes as stipulated by the AIC/MOH and management.
  • To engage the community seniors living within the vicinity of Bukit Batok Nursing Home.
  • To keep the community seniors healthy and independent in the community as long as possible.
  • To refer the community seniors to appropriate healthcare services or agencies as required.

2. ROLES AND RESPONSIBILITIES

2.1 Programmes and Activities

  • To conduct outreach activities to know our seniors in the assigned service boundary, and to support any outreach events and programmes.
  • To conduct/support the implementation of active ageing programmes for the targeted seniors, and encourage active participation from the seniors.
  • To provide feedback for re-evaluation of the activities after each run with the team.
  • To engage the seniors, get to know them and be the resource person should the seniors need other services to stay independently and active in the community.
  • To deliver Befriending and Buddying to more vulnerable seniors in addition to Engagement of those seniors.

2.2 Relationship Management

  • To engage different community partners, build effective and sustainable partnerships with them to serve the needs of the community seniors.

2.3 Community Outreach

  • To be actively involved in Community Outreach efforts to engage seniors – which could take place after office hours or over the weekends.

2.4 Recruitment of Members

  • To promote and advertise the AAC programmes to seniors in designated blocks.
  • To assess and recruit new seniors as members.

2.5 Volunteer Support

  • To work closely with volunteer management team in screening, recruiting, engaging and supporting of volunteers for activities to deliver a wonderful experience for seniors and volunteers.

2.6 Administrative and Logistical Support

  • To provide administrative functions, such as data entry or record keeping, and logistical support in ensuring the quality of the programmes and volunteer experiences.

2.7 Data Tracking and Report Submission

  • To assist in data collection and ensure forms are duly filled as required in Transitional Grant / Eldercare service requirement for reports to be submitted on time to MOH.

2.8 Alert Alarm System for certain rental flats (if applicable)

  • Effective monitoring of the Alert Alarm System (AAS) installed in seniors’ house. Attend to the needs of seniors.

2.9 Any other responsibilities which assigned by the reporting officer.

 

Job Requirements

  • Minimum "O" level & above
  • Preferably to have related qualification in working with the seniors or in the social service sector.
  • Preferably to have at least 1-2 years of experience in direct operational work (especially in Eldercare), such as conducting and evaluating programmes or activities and needs assessment.
  • Preferably has some prior experience in stakeholder management especially in working with community partners.
  • Proficient in Microsoft Word, Excel and Powerpoint.
  • Good interpersonal skills, a team player, yet able to work independently.
  • Good communications skills, preferably bilingual with dialects.
