Pharmacist
Job Responsibilities
- Provide operational support and leadership within pharmacy services to ensure medication safety, efficiency, and regulatory compliance.
- Deliver specialized pharmacotherapy counselling, with focus on chronic disease management, patient safety, and quality of life.
- Coordinate daily workflow to ensure timely, accurate, and safe medication supply.
- Review and validate prescriptions for legality, completeness, accuracy, and clinical appropriateness.
- Resolve medication-related issues with prescribers, with escalation where necessary.
- Ensure accurate dispensing, labelling, repackaging, and compounding in compliance with regulatory requirements.
- Perform medication reconciliation across transitions of care and resolve discrepancies with proper documentation.
- Identify, prevent, and resolve medication-related problems through active review and optimisation of therapy.
- Provide medication counselling, education, and guidance to patients, caregivers, and healthcare professionals.
- Support discharge processes including counselling, adherence programmes, and post-discharge issue resolution.
- Assist in inventory management including stock control, storage, expiry monitoring, and discrepancy investigation.
- Support procurement activities including vendor coordination, purchasing, and cost management.
- Utilise clinical systems for documentation, reporting, and troubleshooting.
- Provide training, mentorship, and competency development for staff and trainees.
- Participate in quality improvement, audits, and research initiatives.
- Assist in managing medication incidents, supply issues, and service gaps.
- Ensure compliance with legal, professional, and organisational standards at all times.
- Recognise and proactively manage risks related to pharmacy operations, supporting organisational risk management and patient safety efforts.
Job Requirements
- Degree in Pharmacy.
- Registered with the Singapore Pharmacist Board with a valid patient care practising licence.
- At least 2 years of relevant experience; prior hospital, clinical, or retail pharmacy experience will be preferred.
- Strong clinical knowledge and understanding of pharmacotherapy.
- Demonstrates a lifelong learning mindset and commitment to continuous professional development.
- Meticulous with good organisational, analytical, and decision-making abilities.
- Effective communication and interpersonal skills.
- Strong problem-solving and conflict resolution capabilities.
- Ability to supervise, guide, and mentor junior staff and trainees.
