Home Care Nurse

Job Responsibilities

• Delivery of safe care and sound clinical decision making
• Assessing and documenting observations of clients condition at each visit
• Initiating/ reviewing nursing care plan based on assessment made
• Maintaining accurate nursing documentation
• Performing necessary nursing procedures such as wound dressing, urinary catheter insertion, nasogastric tube insertion, medication packing and etc.
• Conducting regular evaluation of care provided for home care services
• Provide caregiver training and education to client including families on home health care procedures and strategies
• Reporting of any problem incidents encountered in a client’s condition and/or in handling clients and/or caregiver to Nurse Lead
• Participate in project work to improve the quality of care and services to patients
• Identify service gaps in response to clients’ needs and make recommendations to improve service provision of home care
• Participate in integrated Home Health project to provide palliative care services for home health clients which includes telesupport

Job Requirements

• Diploma in Nursing
• 2 years of experience in community nursing/ home care
• Those with palliative care / oncology training or background will be an added advantage
• Capable of performing a wide range of complex nursing procedures without immediate on-site supervision
• Ability to work independently with strong decision-making skills in a home setting
• A positive, empathetic personality with a genuine passion for client care
• Proficient in MS Office applications
