Job Responsibilities

  • Manage home care client database to ensure completeness and accuracy.
  • Process home care referrals through AIC referral system as point of liaison.
  • Perform basic financial assessment of cares needs for home care clients and conduct home visits where required.
  • Coordinate internal and external referrals of services required (e.g referring home personal care client for home nursing or medical social worker.
  • Coordinate with volunteers to engage home care clients meaningfully.
  • Respond to complaints and feedback from clients and next-of-kin.
  • Manage and schedule deployment of care staff/clinicians for clients based on their preferred date, time and frequency of service etc
  • Check and verify invoices based on the services rendered every month.
  • Collect relevant and accurate data for reporting to management and regulatory authorities. 
  • Identify service gaps in response to clients' needs and make recommendations to improve service provision of home care.
  • Process senior mobility fund (SMF) applications for clients upon forms completion by nurse/therapist.
  • Verify and submit quarterly SMF claim submission.
  • Generate monthly invoices for client services and liaise with Finance/Business Office to ensure transactions are accurate.
  • Verify and process monthly invoices from vendor/service partners to ensure smooth payment of services.
  • Verify and submit invoice details and attendance in ILTC system for subvention claims every month.
  • Verify and submit quarterly ILTC subvention report.
  • Submit medisave claims for clients every month.
  • Any other duties as assigned by reporting officer.

Job Requirements

  • At least Diploma in Nursing / Allied Health/ Psychology/ Counselling/ Social Work/ Community & Social Service (Eldercare) is preferred.
  • At least 1 year in ILTC sector.
  • Proficient in Microsoft Word, Excel, Powerpoint and Outlook.
  • Excellent time management skills, well organized and outcome focused. 
  • Able to work in fast paced and dynamic environment.
